Fix Alertmanager Caddyfile route pointing to Grafana port (#13) 
Alertmanager reverse_proxy was pointing to :3000 (Grafana) instead of
:9093 (Alertmanager). Copy-paste artifact. Fixed in both the Caddyfile
and the template.
